Source Wellness' website (which is hosted by Squarespace and who's Privacy Policy you can view here) stores temporary cookies. Cookies are pieces of information that a website sends to your computer while you are viewing the website. These pieces of information allow the website to remember important information that will make your use of the website more useful. You can alter web browser settings to turn off cookies or allow the computer to display a warning each time a cookie is being received.

Sharing Information
Source sometimes employs third parties to perform services for us. For example, we hire agencies to help book appointments, take payments and do security scans.
- Third-Party Websites: Source’s websites may sometimes include third-party platforms and links to other websites. Source has no control over the privacy practices of sites outside our domain. If you follow a link to another website, please read that site's privacy policy.
- Booking an Appointment: When you choose Book Now on the Source Wellness website, you are automatically transferred to a third-party website to schedule your appointment and receive notifications regarding your appointment. In doing so, you agree to the Privacy Policy of Booker which can be reviewed here.
- Insurance Verification Form: When you submit the Insurance Verification Form on the Source Wellness website, any information you enter (ie.name, email, phone number, insurance provider, insurance policy number, date of birth and referred practitioner) are collected. Your information is shared with our medical billing company in order to verify your coverage.
